BOOK RENTAL FEES
The book rental fee is $60.00.
Students losing or defacing a book will be charged the replacement
cost of the text. If a student leaves the district during the
school year, refunds are issued as follows: Leaving during
the first quarter – 75%; leaving during the second quarter
– 50%; leaving during the third quarter – 25%; leaving
during the fourth quarter – no fees refunded.
PHYSICAL EXAMINATIONS
Physical
examinations must be taken and the form turned in to the office or
coach prior to practice and participation in sports
and cheerleading (August 15th). If students have not yet had a
physical, make arrangements with your doctor. Forms are
available in the WHS office or on the internet at kshsaa.org.
Alma Health Care Clinic also has forms. Both student
and parent must sign the physical form.
COMBINATION LOCKS
Combination padlocks are available to use
free of charge. Students are highly encouraged to utilize the
lock for safe keeping of belongings or school equipment
assigned. The padlock must be returned in good
condition at the end of the year or a replacement fee will be
charged.
STUDENT INSURANCE
USD
329 Mill Creek Valley purchases a student accident insurance
policy. This insurance covers student accidents during the
school day only. The district no longer purchases additional
coverage for student athletes. Coverage for student athletes
is available for purchase through the school district or may be
acquired through your insurance agent. The coverage is
limited, but will coordinate benefits with your insurance. If
your child does not have health insurance, please be sure to ask
for information about the state children’s health plan
(HealthWave) for uninsured children.
LOCKERS
Students are
asked to keep lockers in good condition and not to apply stickers
or tape to any part of the locker. Lockers are to be kept
free of clutter and inappropriate pictures. Students
are not to switch from the locker assigned to them without
notifying the office.
PHYSICAL EDUCATION STUDENTS
P.E.
students are required to wear shorts and a shirt in good condition,
the correct size, and washed regularly. Inappropriate or
offensive T-shirts will not be allowed.
Each student
should have a good pair of gym shoes to be worn only for gym class
in order to maintain the gym floor and keep it in good
condition. Street shoes are not to be worn on the gym floor
at any time. Shoes that allow marks or streaks on the gym
floor will not be allowed.
LUNCH/BREAKFAST POLICY
Student lunches are $2.00 per meal, which
includes a carton of milk. Breakfast is $1.25. Extra milk is
$.25 per carton. Extra servings are $.75 for lunch and $.50
for breakfast. Ala carte breakfast items will be
available also, with prices ranging from $.25 to $1.50. Meal
money should be turned in to the office before 11:00 daily.
Our school participates in the “Offer
versus Serve” lunch program. “Offer versus
Serve” is a federal regulation designed to reduce food waste
in the lunch program allowing students to choose only those foods
that they intend to eat. Each day the students will be
offered the complete lunch including a serving of: (1) milk;
(2) bread/bread alternate; (3) meat/meat alternate; (4) fruit;
& (5) vegetable. Three servings of fruits and vegetables
may be a combination of a serving of fruit and a serving of
vegetables, or two different fruits, or two different
vegetables.
The five food items comprise the school
lunch pattern. Under the “Offer versus Serve”
program, each student must select at least three food items.
The price per meal remains the same whether the student selects the
minimum three different food items or all five.
